On December 18, 2006 Michael Johnson of Northport, FL is believed to have caused a three vehicle accident when he swerved his BMW across traffic to exit onto McDuff Avenue, in Jacksonville, FL.

According to investigators Johnson's BMW struck a van and then struck a truck. The BMW and truck flipped on their roofs, blocking the off ramp for several hours. Johnson was treated and transported to a local hospital where he died. Carl Randall, driver of the truck only suffered minor injuries. No one in the van was injured.
